<p>INSTRUCTIONS FOR THE FORM</p> <p>&nbsp;</p> <p>In order to insert a request, it is necessary to select &lsquo;start new request&rsquo; at the end of this page, and then &lsquo;identify&rsquo;</p> <p>&nbsp;</p> <p>To be identified, insert the UNIMI credentials (e.g. name.surname@studenti.unimi.it), provided to you by the CAS system of the university (authentication system)</p> <p>&nbsp;</p> <p>If you do not have these credentials, register to the system &ldquo;register as external user&rdquo;. Every login after this will allow to use the credentials used when you registered to the system. To access the requests, just click on &lsquo;access your requests&rsquo; in the top right corner, and therefore login with your UNIMI credentials.&nbsp;</p> <p>The form must be filled in in all sections except those which are described as &lsquo;optional&rsquo;. Remember to always click on save and proceed, also if you do not fill in the &lsquo;optional&rsquo; sections. This allows to the registration to succeed.&nbsp;</p> <p>&nbsp;</p> <p>Once you are done filling in the form, you will be able to see and print a summary of the data inserted, and you will be able to proceed to the automatic forwarding to the respective office.
