NOTE: When you first access the online form, CLICK "NEW APPLICATION" AND THEN "IDENTIFY YOURSELF" AFTER VIEWING THIS SCREEN.
IN THE IDENTIFICATION SCREEN, log in using your University credentials (e.g. undergraduate or graduate students); if you do not have University credentials, CLICK "Register as an external user" and follow the instructions.
The next time you log in, you can use the username and password chosen when registering. Remember to select "External user" as "User type".
To access your applications, click on MY APPLICATIONS, top right, and log in with your credentials.
Fill out all the sections of the form, unless otherwise indicated. Before moving from one section to the next, click on the Save and continue button. You have to complete the form in one session, or you will lose the data you have entered.
Once you have filled in the form, you can view and print a summary and automatically submit it to the competent office. The system will send two messages to the institutional email address typed in the form:
