FORM - INTERNATIONAL MOBILITY AND SAFETY

<p><span style="font-size:14px"><strong>General Information</strong><br /> To be completed shortly before departure, even for trips lasting only one day.<br /> Filling out this form does <strong>not</strong> replace the authorisation required to carry out the mobility period, which must be obtained in advance through the relevant structure/academic authority or following selection through mobility calls, according to the procedures outlined below:</span></p> <ul> <li><span style="font-size:14px"><strong>Students:</strong> participation in mobility calls and completion of documents after selection.</span></li> <li><span style="font-size:14px"><strong>PhD students:</strong> obtain authorisation from the Doctoral Course Academic Board. The authorisation is obtained by filling out, before departure, the form available on the webpage [[<a href="https://www.unimi.it/it/studiare/frequentare-un-corso-post-laurea/dottorati-di-ricerca-phd/borse-e-tasse/maggiorazione-periodi-allestero" target="_blank">Maggiorazione periodi all&#39;estero | Universit&agrave; degli Studi di Milano Statale</a>]. The form, signed by the Coordinator, must be sent via <em>InformaStudenti &gt; Postgraduate &gt; PhD Programmes</em>.</span></li> <li><span style="font-size:14px"><strong>Specialisation students:</strong> authorisation to go abroad must be requested from the School&rsquo;s Management and approved through the minutes of the School Council.</span></li> </ul>
<p><span style="font-size:14px"><strong>INSTRUCTIONS FOR COMPLETING THE FORM</strong></span></p> <p><span style="font-size:14px">To submit a request, you must select <strong>&ldquo;START NEW REQUEST&rdquo;</strong> at the bottom of this page and subsequently <strong>&ldquo;IDENTIFY YOURSELF&rdquo;</strong>:</span></p> <p><span style="font-size:14px">To identify yourself, you must enter your UniMi credentials (e.g., nome.cognome@studenti.unimi.it) issued by the University&rsquo;s authentication system (CAS). If you do not have these credentials, you must register with the system (<strong>&ldquo;REGISTER ON THE PORTAL AS AN EXTERNAL USER&rdquo;</strong>). For subsequent access, you can use the credentials chosen during the system registration. The form requires mandatory data entry in all sections, unless otherwise indicated. Before moving from one section to the next, you must click the <strong>Save and proceed</strong> button. You can choose to click <strong>Save</strong> if you do not plan to complete the form in a single session or if you prefer to complete the sections out of the proposed sequence. A final screen will confirm the automatic submission of the application to the competent office. Furthermore, the system will send two communications to the student&#39;s institutional email address:</span></p> <ul> <li> <p><span style="font-size:14px">Notification of successful application submission;</span></p> </li> <li> <p><span style="font-size:14px">Notification containing the official registration number (protocol number) for the application sent. This second communication may arrive several hours after the first notification and serves as proof that the application has been taken over by the competent office.</span></p> </li> </ul> <p><span style="font-size:14px"><strong>N.B.:</strong> In the event of partial completion of the application, it is possible to access the link provided in the call for applications again and enter the <strong>My requests</strong> section from the introductory page: you will be asked to enter your institutional email address, then to select the CAS option and enter your University credentials. The procedure will show the application(s) not yet submitted and will offer the possibility to delete them or resume their completion (if it is possible to submit multiple applications for the same selection).</span></p>
