<p><strong>HOW TO FILL IN THE FORM</strong></p> <p>To submit your request, select &lsquo;SUBMIT A NEW REQUEST&rsquo; at the bottom of this page&nbsp; and then &lsquo;LOG IN&rsquo;:</p> <ul> <li>To log in, enter your UniMi credentials, i.e. @xxx.unimi.it, provided by the University Identification system (<em>CAS</em>).</li> </ul> <p>You must fill in all the sections included in the form. For the form to be correctly submitted, you must always <strong>click on &lsquo;Save and Continue&rsquo; before moving on to the next section.</strong></p> <p>Instructions are available both in Italian and in English: you can switch language by clicking on the corresponding flag in the top right-hand corner of the webpage.</p> <p><strong>After filling in</strong> the form, it will be possible for you to check the <strong>summary page</strong> reporting the data you entered and then forward it automatically to the relevant office. You will be sent two notifications at the university email address you entered in the form:</p> <ol> <li>a notification confirming that your request was submitted successfully</li> <li>a notification reporting the registry number of your request.</li> </ol> <p>You must fill in all the sections included in the form, save as differently specified. For the form to be correctly submitted, you must always click on &lsquo;SAVE AND CONTINUE&rsquo;. The form must be completed all at once, otherwise data will be lost.</p> <p><strong>After filling in</strong> the form, it will be possible for you to <strong>check and print out</strong> the summary page reporting the data you entered and then forward it automatically to the relevant office.
